Job Summary:

In this position, the Social Worker provides comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical. Primary assignment will be in the Intensive Care and Inpatient Medical/Surgical Units with additional duties as assigned at the Community Campus. In addition, the social worker addresses social determinants and complex issues. The Social Worker assigned to the medical inpatient units assists with throughput and capacity. They assist and collaborate to make referrals with multiple community agencies to meet the needs of the patients and families. The social Worker works closely with the multidisciplinary team.  This position is part of the interdisciplinary team that recommends and delivers appropriate social work intervention and support.  Through the use of a wide range of interventions and treatment modalities, the Social Worker contributes to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues which may impede care. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities. Knowledge of Population Health and Social Determinants preferable. 

Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday - Friday 8:30am - 5:00pm with occasional on call/holiday coverage on a rotating basis to meet operational needs
