Job Summary: The Outpatient Family and Adult Medicine Social Worker's primary responsibility is to function as a consultation and referral service to address the psychosocial needs of the patient population served by the  outpatient areas of Upstate Medical University. Through the use of a wide range of interventions and treatment modalities, the Social Worker contributes to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues and social determinants which may impede care.
Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications:

Knowledge and experience using assessment tools, ie: PH Q9 and GAD7 preferred.

Work Days:

Monday-Friday 8:30-5 Occasional on-call/holiday coverage on rotation. Based on operational needs..
