Job Summary: The Social Worker's primary responsibility is to function as a Patient Centered Medical Home (PCMH) social worker at University Internists and the Upstate Family and Preventive Medicine offices at both campuses. This social worker works with individual patients to plan, manage and coordinate care based on need. Patients are identified that may benefit from care management services. The process may include behavioral health conditions, high cost/high utilization, poorly controlled or complex conditions, social determinate of health, and referrals to outside organizations. Through the use of a wide range of interventions and treatment modalities, the Social Worker contributes to the achievement of optimum health, satisfaction and cost-effective care by addressing the behavioral, social and environmental issues which may impede care.
Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday 8:30-5 Occasional on-call/holiday coverage on rotation. Based on operational needs.
