Job Summary: Working within Ambulatory Family Medicine Department, the Registered Nurse will assess and identify patient and family health status, establish a plan of care, implement and evaluate the care provided, in addition to integrating patient and family education into the delivery of care. Professional ambulatory care nurses facilitate continuity of care using the nursing process, nurse triage, interdisciplinary collaboration, and coordination of all appropriate health care services, including available community resources. The position will also supervise, coordinate, and evaluate care provided by LPN's, MOA's, and other technical and assistive personnel. This position will require the RN to cross train and support other sub-specialty clinics. This is a grant funded position. The RN will be dedicated to providing coordinated and comprehensive care managemnt support to adult diabetic patients using SMART CARE model for a period of twelve (12) months.
Minimum Qualifications:

RN with NYS license/registration required. Excellent communication skills, ability to work in a fast-paced, multi-disciplinary environment and BLS CPR certification required.

Preferred Qualifications:

Experience working in an ambulatory setting preferred.
