Job Summary: The Nurse Practitioner/Physician Assistant will join Upstate Pediatrics, a busy pediatric primary care office. Upstate Pediatrics is an NCQA Patient Centered Medical Home. Our team includes five faculty pediatricians, dedicated nurses and front staff, and a panel of rotating third year medical students. The NP/PA will work in collaboration with physicians in providing diagnostic and treatment services to pediatric patients, age newborn to 21 years. Will assess, diagnose, and develop a treatment plan in accordance with practice protocols. Will be responsible for obtaining complete medical histories, performing physical examinations, ordering appropriate diagnostic tests and making referrals for specialty consultations and care. NP/PA will be responsible for maintaining accurate and complete medical records in the electronic medical record system.
Minimum Qualifications:

NYS licensed/registered NP or PA required (NYS NP specialty area of certification must be consistent with practice). Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and Master's Degree) (eligibility criteria for PA requires graduation from an accredited PA program and National Certification through NCCPA). Initial certification must be present or obtained within 6 months of hire. Current CPR certification through the American Heart Association or the American Red Cross is required.
