Job Summary: NP will be responsible for obtaining complete medical histories, preforming physical examination, diagnosing and treating simple and complex medical conditions, ordering appropriate diagnostic tests and making referral for specialty consultations and care. Responsible for maintaining accurate and complete medical records. Experience with electronic health records a plus.
Minimum Qualifications:

NYS licensed/registered NP or PA required (NYS NP specialty area of certification must be consistent with practice). Must have NPI number and be Medicare Enrolled with Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within 6 months of hire (eligibility criteria for NP requires National Certification through AANP or ANCC, and Master's Degree) (eligibility criteria for PA requires graduation from an accredited PA program and National Certification through NCCPA). Initial certification must be present or obtained within 6 months of hire. Current CPR certification through the American Heart Association or the American Red Cross is required.

Preferred Qualifications:

Family Nurse Practitioner with minimum 2 years Family Practice experience. the candidate will work in a busy Family Medicine practice on the Onondaga Nation delivering comprehensive patient care in an ambulatory outpatient setting. This person will work with physicians in providing diagnostic and treatment service to individuals. Will assess, diagnose and develop a plan of care in accordance with practice protocols.

Work Days:

Monday Tuesday Thursday 8 a.m.-5 p.m. No rotating or weekend shifts.
