The Upstate Triage & Transfer Center is a RN supported, exciting unit that is on the cutting edge of the healthcare system's current direction. This department of nurses, fields telephone calls for health information and injury and illness triage requests from the community as well as contracted offices and group homes. This dynamic department is the point of contact for referring facilities that need to transfer a patient to either one of our campuses for a higher level of care or a specialty service. We are the face/voice of Upstate and the first person that a referring facility talks to when requesting a telemedicine consult for a patient in their Emergency room or as an Inpatient. We perform some patient satisfaction and transitioning back into the home surveys for patients that have been treated at one of our many locations. We offer after hours triage for offices and groups. We are the nursing support when everyone else is closed or off.
