The Admission Liaison/Coordinator is responsible for coordination of all admissions to University Hospital Inpatient Acute rehabilitation units, Transitional Care Unit. Responsible for screening, insurance authorization, and coordination of necessary steps to admit patient to the rehabilitation unit and TCU based on regulatory requirements and patient medical and functional status. Collaborates with consulting services in conjunction with Physical Medicine and Rehabilitation Medical Staff, TCU Attending Medical Physician to admit patients appropriate for rehabilitative and TCU care. Responsible for maintaining admission databases and ongoing quality data monitoring. Responsible for establishing relationships and outreach to internal and external referral sources. Responsible for coordinating care with the inpatient rehab team as well as consulting/referring services. Responsible for keeping current with regulatory and requirements as it relates to inpatient rehabilitation facilities and SNF.  Obtain Insurance authorizations as needed. Assist PPS coordinator with data collection for completion of IRF Patient Assessment Instrument. Must demonstrate ability to effectively communicate, establish and maintain effective working relationships with patients, families, insurance companies, internal stakeholders, outside referring hospitals, and members of the health care team.
