SUNY Upstate has an exciting opportunity for a compassionate Advanced Practice Provider interested in providing cutting edge primary care to adolescent patients. The Adolescent APP will be a member of the Adolescent Care Team at the Upstate Pediatric and Adolescent Center at the Nappi Wellness Institute.
Adolescent needs include:
Contraceptive care, hormone therapy, body image concerns, and eating disorders treatment.
Clinical responsibilities include:
Acute care visits, continuity visits and other patient specific ambulatory visits based on individualized patient needs.
Overall Responsibilities include:
Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care.
Assessing & evaluating patients.
Ordering appropriate laboratory testing & diagnostic studies.
Arranging for referrals, testing, consultation & therapeutic services.
Maintaining outstanding relationships with families, referring physicians & all members of the health care team.
SUNY Upstate APP’s care for patients, families and the community and a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution and exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.

NYS licensed/registered NP or PA required.
Current National Certification consistent to the clinical practice also required.
Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
CPR-BLS Certification required at the time of appointment.
Must successfully pass pre-employment and credentialing clearances.
Adolescent, LGBTQ Care, and Ambulatory experience preferred.
Monday – Friday, Days, May include some evening hours based on clinic needs.
