The After Office Hours Care Manager is an integral member of the after-hours mental health program, responsible for coordinating and supporting patient-centered behavioral health care. This role partners closely with providers, patients, and families to monitor treatment progress, address barriers to care, and promote engagement, education, and adherence to treatment plans.

What You’ll Do

  • Coordinate and support mental health care services in collaboration with behavioral health providers and clinical staff
  • Monitor and track patient treatment responses, including changes in symptoms, side effects, and potential complications
  • Educate patients and family members on available resources
  • Encourage patient engagement in care and promote adherence to prescribed treatment plans
  • Facilitate referrals for additional services and support based on patient needs
  • Support medication management by monitoring adherence and assisting with access to medications
  • Assist in scheduling and coordinating patient appointments and managing communication (e.g., phone calls) for the mental health care team
  • Identify and address psychosocial barriers that may impact a patient’s ability to access or maintain treatment
  • Provide ongoing support to patients and families to improve overall care outcomes and continuity of care

Minimum Qualifications:

Candidates will be considered with a combination of the following credentials:

Bachelors degree in Psychology or a related field and two years of healthcare-related experience;
OR
Bachelors degree in Nursing, with NYS RN licensure/registration (or eligibility) and two years of post-RN experience;
OR
Masters degree in Social Work from a CSWE-accredited program, with LMSW, LCSW, or eligibility, and three years of relevant post-masters experience (preferably in a hospital and/or healthcare setting).

*Title and salary will be commensurate with qualifications.

Preferred Qualifications:

Preferred candidate will have experience in behavioral healthcare setting working with a multidisciplinary team and/or medical setting. Demonstrated prior interactions with patients in medical setting preferred.

Work Days:

  • Evening and weekend shifts
  • Monday–Friday: Between 1:00 PM – 9:30 PM
  • Weekends: Between 11:00 AM – 7:30 PM
  • Flexible scheduling within these ranges

Salary Range: $69,000+ (salary based on full time equivalence and education & experience)
