Job Summary:

The Assistant Director of Case Management assists the Director of Case Management with responsibility and accountability for managing of the case management staff: hiring, terminating, supervision, scheduling, orientation of new staff, submitting F1s and oversight of position control and assist in managing the day to day operations. Attend the following meetings, daily operations huddle, long length of stay at both campuses, escalation, post acute partnership meeting as needed. Co facilitates case management staff meetings and provide administrative coverage for Director of Case management. Specific duties would include assisting with coordination of strategic and operational case management initiatives, customer and staff satisfaction, quality management, certification and accreditation activities and development of professional practice activities. Oversight of SA system as it relates to case management.

Minimum Qualifications:

Bachelors degree in Nursing, NYS Licensed/Registered (or eligible) RN and a minimum of 5 years post-licensure acute care RN experience required. Minimum of one year Nursing leadership experience required. Prior Case Management experience and demonstrated competency in teaching/learning process, organizational skills and computer literacy required. Excellent written/oral communication, leadership, organizational, interpersonal and analytic skills required. CPR-BLS certification required by time of appointment.

Preferred Qualifications:

National Certification in Case Management, Care Coordination or Transitional Care strongly preferred. Master's degree in Nursing preferred.

Work Days:

Monday- Friday: Days with some weekend responsibilities
