Job Summary:
This clinical pharmacist will primarily provide pharmaceutical care for adult, malignant hematology patients at the Upstate Cancer Center (DT Campus). This will include playing a lead role in the management of all pharmacy aspects of Cellular Therapies (CT) and CAR-T cell therapy program maintenance. This individual will be responsible for establishing relationships with the adult malignant hematology providers and service representatives. Routine responsibilities will include but are not limited to: Daily patient care as part of an outpatient multi-disciplinary team, building oncology treatment plans, maintenance of policies/procedures related to malignant hematology and oncology services, formulary management, and patient counseling. Based on operational needs, this pharmacist may contribute inpatient malignant hematology / BMT service (Team #8) activities to maintain continuity of care.
As a member of the pharmacy department, all pharmacists should also be able to perform the following functions as needed by the department: Interprets medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. Pharmacist prepares compounds and dispenses medications as needed for inpatient and ambulatory care use. This includes the preparation of injectable sterile products. Dispenses Pharmacy related items. Performs duties in all areas of Pharmacy to include inpatient, ambulatory care, and satellites. Provides information to health care practitioners to promote the rational use of medications. Provides drug monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy and procedures for hospital staff. Participates in teaching programs for hospital staff. Participates in Pharmacy Departments Quality Assurance Programs.
Minimum Qualifications:
B.S. in Pharmacy or Pharm.D and NYS Pharmacist license and registration (or eligible) required.
Preferred Qualifications:
PGY-2 Pharmacy Residency in Oncology, Board Certification in Oncology Pharmacy, Previous bone marrow transplant, CAR-T or Malignant Hematology experience preferred.
