Job Summary:

The Director of Case Management assumes the overall responsibility for the implementation of the vision, mission, plans, and standards of the organization and patient care services within the division of the Transitional Care Department. Specific responsibilities include the management and evaluation of clinical operations, patient and employee experience, quality management, standards, certification, and accreditation activities within the Department of Case Management. Working in collaboration with the Director and Associate Director of Transitional Care, assist in establishing goals that align with the organizational strategic plan.

Minimum Qualifications:

NYS licensed RN (or eligible), Bachelor's degree in Nursing and a minimum of 5 years progressively responsible nursing experience, preferably in case management and in an acute care environment, excellent written/oral communication, leadership, organizational, interpersonal and analytical skills required.

Preferred Qualifications:

National certification in Case Management or Care Coordination and Transitional Management is strongly preferred. MSN and prior nursing leadership experience strongly preferred.

Monday-Friday, Days
