Job Summary:

The HELP Nurse Principal Advancement Leader is a professional registered nurse with leadership responsibility as part of the Hospitalist Escalation Team for coordinating and facilitating efficient, safe care by coordinating with multidisciplinary teams (physicians, consultants, ancillary services, social work, case management, Outpatient services, etc.). The goal is to optimize hospital throughput, appropriate length of stay (LOS), and support quality care progression through the continuum. The HELP Nurse Principal Advancement Leader will serve as a leadership resource to facilitate prompt discharges through collaboration with services across the care continuum, driving better utilization of services, continuity of care across inpatient and outpatient services, and improving quality outcomes.

Minimum Qualifications:

NYS licensed RN (or eligible), Bachelor of Science in Nursing required, with a minimum of three years of relevant experience, including at least (1) one year in an acute care setting.

Preferred Qualifications:

Master's degree preferred. Previous care transition/case management preferred. Leadership experience

Monday-Friday, Days
