Job Summary:
SUNY Upstate Medical is seeking a highly skilled and motivated Part-Time Advanced Practice Provider who will provide care to patients at the Breast Care Center at Wellspring at Upstate Community Campus with possible rotation at 550 Harrison Breast Care Center. The Advanced Practice Provider will provide High Risk Breast Care services with cross coverage for comprehensive Breast Care and Endocrine as well in the same department
Key Responsibilities:
- Provide expert level clinical care & coordination of care via a full range of services that will improve quality of patient care.
- Assess & evaluate patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services.
- Maintain outstanding relationships with families, referring physicians & all members of the health care team.
As an APP, you will be an integral member of the healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.
Minimum Qualifications:
- Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program).
- NYS Licensed/Registered NP or PA.
- Current National Certification consistent to the clinical practice.
- Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
- CPR-BLS Certification required at the time of appointment.
- Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
- 2-3 years of APP experience preferred.
- Surgical, Oncology, and/or Women’s Health experience preferred.
Work Days:
Monday, Thursday and Friday, Days; No Holidays, Weekends, or Call
