Job Summary:

Inpatient Palliative Care Practice Provider will provide Full Time care to patients who require specialized Palliative Care Consultation. This position is located at SUNY Upstate Downtown with rotation to the Community Campus.

Responsibilities include but not limited to:

  • Consultative services to assure identification and communication of patient goals of care.
  • Symptom management recommendation and care.
  • Providing acute and chronic pain management through appropriate assessment.
  • Evaluation and treatment, education, and guidance on the principles of palliative care.
  • Institutional leadership in all process improvement activities related to palliative care.
  • Maintaining outstanding relationships with families, referring providers and all members of the health care team.

SUNY Upstate APP’s care for patients, families and the community through assessment, planning, prescribing and evaluation interventions. An APP is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution and exemplifies the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures. 

Minimum Qualifications:

  • Master’s Degree as a Nurse Practitioner or Physician Assistant.
  • NYS licensed/registered NP or PA required
  • Current National Certificate consistent to clinical practice is also required.
  • Have an NPI number and be Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
  • CPR-BLS Certification required at the time of appointment.
  • Must successfully pass pre-employment and credentialing clearances.

Preferred Qualifications:

  • 1-2 years Inpatient experience
  • Palliative Care experience

Work Days:

  • Monday – Friday with rotation of weekends and holidays.
