Job Summary:

Advanced Practice Provider will serves as a Full-Time APP, providing care to patients within the Joslin Diabetes Center at SUNY  Upstate’s Community Campus.  The APP will provide adult diabetes management in the outpatient setting.

Responsibilities include:

  • Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients.
  • Ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services.
  • Maintaining outstanding relationships with families, referring physicians & all members of the health care team.

Minimum Qualifications:

Master’s Degree as a Nurse Practitioner or Physician Assistant.

NYS Licensed/Registered NP or PA.

Current National Certification consistent to the clinical practice. 

NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications:

Diabetes Management. 

Experience in an Ambulatory setting.

Work Days:

Monday - Friday, Days
