Job Summary:

The Nephrology Advanced Practice Provider will serve as a full time APP to provide care to patients at Upstate Community Hospital. The APP will provide inpatient management of the nephrology patient. Responsibilities include providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services, and maintaining outstanding relationships with families, referring physicians & all members of the health care team.

Minimum Qualifications:

NYS licensed/registered NP or PA required. Master’s Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).

1-2 years Inpatient medicine experience.

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications:

Nephrology & Dialysis experience preferred.

Work Days:

Monday – Friday days with possible evening and weekend rotation
