Job Summary:
SUNY Upstate Pediatrics is moving to a new location and expanding our Advanced Practice Provider coverage. In support of our continued growth and expansion we have an exciting opportunity for a highly motivated APP interested in providing cutting edge Primary Care to Pediatric Patients, Birth to 21 years, at our Upstate Pediatrics location in Baldwinsville.
Key Responsibilities:
- Clinical- routine well childcare, acute care visits, and other patient specific ambulatory visits based on individualized patient needs.
- Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care
- Assess and evaluate patients, order and interpret laboratory tests & diagnostic studies, and arrange referrals, consultations, and therapeutic services.
- Build and maintain strong relationships with patients, families, referring physicians, and all members of the health care team.
As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.
Minimum Qualifications:
- Master’s Degree as a Nurse Practitioner or Physician Assistant.
- NYS Licensed/Registered NP or PA.
- Current National Certification consistent to the clinical practice.
- Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
- CPR-BLS Certification required at the time of appointment.
- Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
Pediatric and Ambulatory experience.
Work Days:
Monday – Friday, may include some evening hours based on clinic needs.
