Job Summary:

The Perinatal Care Manager is responsible for managing incoming referrals and coordinating care for high-risk perinatal patients. This role ensures timely triage, seamless communication among obstetricians, specialists, and support staff, and continuity of care to optimize maternal and fetal outcomes. The RN addresses patient needs based on age, physical, psychosocial, cultural, and educational factors, enhancing clinical flow, patient safety, satisfaction, and maternal-fetal outcomes.

Minimum Qualifications:

NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field and a minimum of three (3) years acute care, transitional care, or ambulatory experience OR Associates degree in Nursing and five (5) years of acute care, transitional care, or ambulatory experience required. CPR-BLS certification.

Preferred Qualifications:

Bachelor's degree in Nursing or healthcare-related field and prior care management experience, ambulatory nursing experience and coordination for high-risk obstetric populations preferred.
