Job Summary:

SUNY Upstate Pediatrics is seeking a Program Case Manager who will serve as an important member of the Patient Services Team, working with the medical team to provide excellent patient-centered care to address social determinants of health which affect health, functioning and quality of life outcomes, working with patients enrolled in the program with the goal of providing resources and referrals to address health disparities and inequities.  Additionally, the Program Case Manager assists with case finding, and supports outreach and program promotion.

The primary responsibility of this position is to provide supportive services to patients enrolled in the YACAP AIDS Institute grant-based program. The YACAP Program serves adolescents and young adults seeking sexual health and HIV prevention services.

Job Responsibilities but not limited to:

  • Complete social determinants of health assessments, and risk assessments with all enrolled patients and document an individualized plan with patients to address identified issues and provide follow-up assistance in reaching goals.
  • Work as part an interdisciplinary team to provide LGBTQ affirming, culturally appropriate case management services to adolescents and adults.
  • Participate in all required and approved trainings.
  • Facilitate appropriate and culturally relevant referrals to community-based programs.    
  • Track and follow up on referrals.
  • Participate in team meetings as required (morning clinic huddle, staff meeting, case conferences and QI).

Minimum Qualifications:

Bachelor's Degree, or Associates Degree and 2 years experience, Or equivalent combination of education and/or experience required.

Preferred Qualifications:

Previous case management experience.

Previous knowledge of HIV prevention and health care.

Experience working with BIPOC and LGBTQ+ communities.

Work Days:

Monday - Friday, 8:00 am - 4:30 pm
