Job Summary:

The RN Navigator is a Registered Nurse who serves as a personal guide and advocate for expectant mothers throughout their pregnancy, delivery, and postpartum journey. Their primary goal is to provide a seamless healthcare experience by coordinating care between various providers and connecting patients with necessary resources.


Core Responsibilities:
"Initial Patient Triage: Conducts early intake assessments to identify medical, social, or financial barriers to care.
"Follow-up Missed AB's:

Chart Review:
" Beta Log
" Labs and testing
" High-Risk Coordination: For complex cases, navigators arrange specialized appointments with maternal-fetal medicine (MFM) specialists, neonatologists, and nutritionists.
" Physiologic Monitoring: Assesses patient data such as blood pressure and fetal heart rates, referring clinical concerns to the primary obstetrician as needed.
" Educational Benchmarks: Provides structured education at key pregnancy milestones, such as explaining lab results at 20 weeks and reviewing birth plans and hospital logistics by 36 weeks.
" Patient Education: Explains pregnancy stages, tests, procedures, and lifestyle choices, providing resources.
" Care Coordination: Liaises between the patient, OB/GYN, specialists (like maternal-fetal medicine), social workers, and other departments.
" Resource Connection: Connects patients with community resources, financial assistance, childcare, nutrition counseling, and support services.
" Barrier Removal: Helps overcome healthcare system hurdles like paperwork, scheduling, transportation, and complex medical information.
" Emotional Support: Offers a consistent point of contact and emotional guidance throughout the journey.
" Advocacy: Serves as the patient's advocate, ensuring their needs are met and decisions are informed.
" Assessment: Evaluates patient status and identifies special needs.
" Communication: Answers questions, clarifies medical terms, and facilitates communication between providers.
" Postpartum Follow-up: Checks in with patients after delivery.
" Prenatal Tracking: tracking all prenatal patients.

Minimum Qualifications:

NYS licensed/registered or eligible RN required. Bachelor's degree in Nursing or healthcare related field three (3) years post licensure care RN experience required. Excellent written/oral communication and interpersonal skills required. Current BLS certification through the American Heart Association or the American Red Cross is required by time of appointment.

Preferred Qualifications:

Three (3) years as a Certified Nurse/Patient Navigator in specialty area preferred.

Work Days:

Monday-Friday variable shifts 7 am to 7 pm
