Job Summary:

The Ambulatory Social Worker Float position will provide Social Work services to ambulatory, primary care and specialty clinics to assist with staff vacancies, leaves, unexpected absences and current uncovered ambulatory clinics Downtown and at the Community campus.  Social Workers provide comprehensive psycho-social assessment, management and support to at risk patients and families. Through the use of wide range interventions and treatment modalities, the Social worker contributes to the achievement of optimum health and satisfaction by addressing the behavioral, social and environmental issues which may impede care.  The Social Worker works collaboratively with the multidisciplinary team to support the needs of patients and families, reduce barriers to care and increase optimal health outcomes. The Social Worker will participate in departmental meetings/in-services, education, and quality improvement activities in addition to other relevant duties as assigned.  A strong candidate will demonstrate flexibility, adaptability, and experience with building rapport quickly with team members, patients and family systems. 

Minimum Qualifications:

L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Fridays, days occasional on call/holiday coverage
