Job Summary:

In this position, the Social Worker provides comprehensive psycho-social assessment, management and support to the patient population served by the Center for International Health. Duties will include consultation through assessment, intervention, planning, and referrals to meet the needs of patients and families. In addition, the Social Worker will actively manage patients through collaboration with multidisciplinary team, coordination of care, monitoring and identifying community services. The Social Worker will also participate in quality initiatives in assigned areas. The candidate must possess strong communication and documentation skills, have knowledge of Social Determinants of Health and available community resources. The candidate will participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

Minimum Qualifications:

LMSW from a Social Work program accredited by the Council on Social Work (CSWE); L.M.S.W., L.C.S.W. or eligible; 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required.

*New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications:

Post-master's experience (preferably in a hospital and/or healthcare environment) preferred.

Work Days:

Monday-Friday, days, with occasional on-call/holiday coverage on rotation based on operational needs.
