Job Summary:
In this position, the Social Worker follows patients and families enrolled in the Healthy Steps for Young Children (Healthy Steps) program at the Upstate Pediatric and Adolescent Center. Healthy Steps is a pediatric primary care program that focuses on skills needed for families to foster healthy child development and lifelong wellbeing, social-emotional development, language and literacy skills, cognitive skills, physical development for children ages 0–3. Candidate will provide comprehensive psycho-social assessment, management and support to patients and families enrolled in Healthy Steps. Duties will include consultation through assessment, counseling, intervention, planning, and referrals to meet the psycho-social needs of patients and families. Candidate must possess communication skills, have knowledge of Social Determinates of Health and available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
Work Days:
Monday-Friday, days with occasional on-call/holiday coverage on rotation based on operational needs.
