Job Summary:
The Social Worker designated to work at the Inclusive Health Services (IHS) Clinic will address the identified psychosocial needs and support to remove identified barriers to care for a diverse patient population. Inclusive Health Services at Upstate Medical University is designated by the NY AIDS Institute as a provider of high quality, comprehensive HIV treatment and prevention. We offer extensive services in HIV care, PrEP and PEP. Inclusive Health Services also provides primary care and hormone therapy for people who identify as LGBTQ, regardless of HIV status. Social Workers provide comprehensive psycho-social assessment, management, and support to at risk patients and families. In addition, the Social Worker addresses social determinants of health and complex issues, working closely with the multidisciplinary team. Duties will include consultation through assessment, counseling, intervention, planning, and referrals to meet the needs of patients and families. Candidate must possess strong communication skills, have knowledge of community resources, and good documentation practices. Additionally, participate in departmental meetings, perform other relevant duties assigned to include coverage for other social workers, back-up responsibilities and meeting operational needs of the department.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry-level (SL2) positions at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of the limited permit.
Preferred Qualifications:
Previous knowledge and experience with a diverse patient population.
Work Days:
Monday-Fridays, days with occasional on-call/holiday coverage on rotation based on operational needs
