Job Summary: The Social Worker’s responsibility is to focus on identifying Multi-Visit Patients (MVPs) using internal data and identified criteria to work closely and collaboratively with Emergency Department (ED), Inpatient Services and community-based partners to develop strategies to decrease unnecessary hospital ED utilization.  In this position, the social worker will Identify utilization patterns, drivers of utilization and develop a plan of care to create alternative resources and sites to meet the identified non-hospital/non emergent patient needs. The Multi-Visit Patient (MVP) Method is an evidence-based, person-centered, locally adaptable and operationally feasible methodology for improving care.  Social Workers provide comprehensive psycho-social assessment, management and support to at risk patients and families. In addition, the social worker addresses social determinants of health and complex issues, working closely with the multidisciplinary team.  The candidate shall gain knowledge of social determinants, population health and drivers of utilization. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families.  Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.  Minimum Qualifications:

MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday 8:30a-5p Occasional on-call/holiday coverage on rotation. Based on operational needs.
