Job Summary:

The RN care management position is responsible for assisting patients and families in navigating their healthcare. Their primary focus is on high-risk patient populations within Transplant. The role will encompass patient navigation related to the assigned patients; working closely with multiple agencies, specialty services, and providers. This role will be responsible for the patient's individual needs as they relate to age, physical, psychosocial, cultural, and educational status.

Minimum Qualifications:

R.N. NYS license or eligible, Bachelor's degree in Nursing or healthcare-related field, plus a minimum of three (3) years RN nursing experience required. CPR-BLS is required at the time of appointment.

Preferred Qualifications:

Experience in ambulatory nursing and care management processes to include patient care goal setting, patient care plans, collaboration with community agencies to address SDOH, and familiarity with the IOTA Model for transplant.

Work Days:

Monday-Friday, Days dependent on unit need
