Job Summary:

The Transplant Surgery Advanced Practice Provider will serve in a Hybrid Inpatient and Outpatient position providing care to transplant patients at the Upstate University Hospital Downtown Campus and at 550 Harrison Transplant Clinic. The APP will work collaboratively with the Transplant Team to provide care to the Pre-Transplant and Post Transplant patients.

Key Responsibilities:

  • Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing.
  • Providing care to post transplant patients, ordering appropriate laboratory testing & diagnostic studies
  • Provide evaluation for pretransplant patients, and arranging for referrals, testing, consultation & therapeutic services.
  • Maintaining outstanding relationships with families, referring physicians & all members of the health care team.

As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.

Minimum Qualifications:

  • Master’s Degree as a Nurse Practitioner or Physician Assistant.
  • NYS Licensed/Registered NP or PA.
  • Current National Certification consistent to the clinical practice. 
  • NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
  • CPR-BLS Certification required at the time of appointment.
  • Must successfully pass pre-employment and credentialing clearances. 

Preferred Qualifications:

  • 2 years of inpatient/Outpatient experience
  • Transplant Surgery experience

Work Days:

Monday-Friday Days, with Rotation to Weekends & Holidays.
