Job Summary:

The Acute Care Hospital at Home Case Manager will facilitate the continuum of care for an assigned number of cases by managing, coordinating, facilitating and monitoring resources to achieve optimal clinical outcomes, utilization of services and financial goals. Duties for Acute Care Hospital at Home - will assist in the identification of appropriate patients, screening, hand-off to partner nursing agency and will follow patients throughout their inpatient stay in the home in collaboration with home care agency. Responsible for making home visits as needed, assisting in monitoring patient progress and assisting with discharge planning from the acute phase to the post-acute phase of the episode of care.

Minimum Qualifications:

NYS licensed/registered (or eligible) RN, Bachelor's degree in Nursing or healthcare related field, plus minimum of three (3) years acute care RN or acute care, transitional care, or chronic care RN/case management experience required. CPR-BLS certification required by time of appointment.

Preferred Qualifications:

Master's degree in Nursing or healthcare related field and prior acute care case management experience, knowledge of Interqual Criteria and/or QI/UR experience preferred. General medical/surgical experience preferred. Three years case management/transitional care experience preferred. Experience with Home care preferred.

Work Days:

Monday-Friday, Days: May require some off shift/weekend/ holiday as needed
