Job Summary:

This pharmacist will operate under our ambulatory care pharmacy services umbrella and primarily provide direct patient care to patients in our Family Medicine Clinics. Major responsibilities will encompass the provision of medication management as part of the care team.  As a member of a diverse ambulatory patient care team, this pharmacist may also be called upon collaborate in the provision of patient care to numerous Upstate Clinics. These clinics include, but are not limited to, the following: adult medicine, family medicine, rheumatology, pulmonary, international health, cystic fibrosis, endocrinology, and neurology. Clinical activities will include, but are not limited to, direct patient care and medication oversight, patient counseling, formulary management, provider/staff/resident/student education, specialty pharmaceutical coordination, and transitions of care coordination. This individual will join a team of ambulatory care pharmacists, providers, and nurses to provide coordinated patient care.

As a member of the pharmacy department, all pharmacists should also be able to perform the following functions as needed by the department: Interprets medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. Pharmacist prepares compounds and dispenses medications as needed for inpatient and ambulatory care use. This includes the preparation of injectable sterile products. Dispenses Pharmacy related items. Performs duties in all areas of Pharmacy to include inpatient, ambulatory care and satellites. Provides information to health care practitioners to promote the rational use of medications. Provides drug monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy and procedures for hospital staff. Participates in teaching programs for hospital staff. Participates in Pharmacy Departments Quality Assurance Programs.

Minimum Qualifications:

B.S. in Pharmacy or Pharm.D., NYS license and registration – or eligible – required; completion of a PGY-1 pharmacy residency or equivalent experience required.  

Preferred Qualifications:

PGY-2 Residency Training in Ambulatory Care, Board Certification, and/or previous ambulatory care pharmacy practice preferred; Eligible to engage in Collaborative Drug Therapy Management (CDTM) according to NY State Requirements.

Work Days:

Predominantly weekdays (possibility for evenings/weekends/holidays exist based upon department need)
