Job Summary:
The Burn Nurse Navigator functions within a highly dynamic, multidisciplinary environment requiring consistent coordination across inpatient, outpatient, and community-based settings to support patients throughout the full continuum of burn care. The navigator maintains an active caseload of approximately 40-60 patients while supporting an annual volume of 250-300 burn admissions, prioritizing high-risk individuals and ensuring timely transitions of care. Core operational responsibilities include daily coordination with the burn team, real-time tracking of discharges and follow-up needs, and structured post-discharge outreach (e.g., post-discharge day 1 and day 7 calls) to identify early complications and prevent readmissions. The navigator must demonstrate proficiency in electronic medical record systems and data tracking tools to document patient interactions, monitor outcomes, and contribute to performance improvement initiatives aligned with American Burn Association verification standards. This role demands strong clinical judgment, advanced organizational skills, and the ability to manage competing priorities while collaborating closely with surgeons, nursing staff, rehabilitation services, and community partners. Additionally, the position requires emotional resilience and adaptability to support patients and families experiencing significant physical and psychological trauma, ensuring no gaps in care and maintaining a high standard of patient-centered, coordinated service delivery. In addition, this role required aftercare education and support to outside agencies that provide care to patients with a burn injury.
Minimum Qualifications:
Registered Nurse (RN) current licensure in New York State. Bachelor of Science in Nursing (BSN) and 3 years of experience in burn, critical care, or trauma nursing. Strong communication and demonstrated experience in care coordination and patient education. Knowledge of aftercare needs for patients with a burn injury.
Preferred Qualifications:
Certified Burn Registered Nurse (CBRN). Master's degree in Nursing or related field. Experience with ABA verification standards. Prior experience in a navigator, coordinator, or case management role
