Job Summary:
The Care Manager is a core member of the Syracuse City School District school-based mental health program, working closely with the students' behavioral health providers and staff. The Care Manager is responsible for supporting and coordinating the mental health care of students, monitors and tracks treatment response for changes in clinical symptoms and treatment side effects or complications. Supports and encourages students and family members to increase knowledge about their mental and physical health conditions, engage in their care, and improve adherence to treatments. Provides education and available treatment options. Facilitates referrals for other needs and supports patient medication management including monitoring adherence and ability to access obtained medications. Assists with managing patient appointments and phone calls for school-based mental health care team, in coordination with support staff. Provides support to students and families where psychosocial barriers interfere with their ability to access or maintain recommended treatments for behavioral health concerns.
Minimum Qualifications:
Bachelors degree in Psychology or related field and two years of healthcare related experience required.
Will also accept RN or a Masters in Social Work from a Social Work program accredited by the Council on Social Work (CSWE); L.M.S.W., L.C.S.W. or eligible, and 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. *Title & salary will be commensurate with qualifications.
Preferred Qualifications:
Preferred candidate will have experience in behavioral healthcare setting working with a multidisciplinary team and/or medical setting. Demonstrated prior interactions with patients in medical setting preferred.
