Job Summary:
The Clinical Case Manager Support Assistant position is needed to help Clinical Case Managers manage the growing administrative and care coordination responsibilities associated with caring for hospitalized patients. As patient needs become more complex and documentation and regulatory requirements continue to increase, Clinical Case Managers are spending more time on administrative tasks that take away from direct patient care coordination and discharge planning. This position will provide support with daily activities such as managing Epic in-baskets, documenting interdisciplinary rounds, completing required patient screenings, coordinating referrals and follow-up appointments, and ensuring required documentation is completed. By assisting with these responsibilities, Clinical Case Managers can focus on patients with more complex medical and social needs, discharge planning, and removing barriers to a safe transition of care. In addition to meeting current operational needs, this position creates a career pathway for employees who are interested in advancing into Clinical Case Management. It provides hands-on experience with care coordination, discharge planning, Epic workflows, and interdisciplinary collaboration while building the knowledge and skills needed to transition into a Clinical Case Manager role in the future.
Minimum Qualifications:
Bachelor's degree in nursing or a healthcare-related field and a minimum of two (2) years of clinical healthcare experience or Associate Degree in Nursing with a minimum of four (4) years of clinical healthcare experience.
Demonstrate effective verbal and written communication skills with patients, families, physicians, and members of the interdisciplinary healthcare team. Proficiency in computer applications, including Microsoft Office and Epic electronic health record (EHR) systems. Strong organizational, critical thinking, and time-management skills with the ability to prioritize multiple responsibilities. Ability to maintain patient confidentiality and comply with HIPAA and organizational policies
Preferred Qualifications:
Familiarity with interdisciplinary care teams and collaborative care coordination. Experience in an acute care hospital/chronic care with care coordination/primary care with care coordination setting. Strong nursing assessment skills with the ability to identify patient needs and communicate effectively to the Clinical Case Manager and interdisciplinary team.
