Job Summary:

Working under the Heart Failure Program Coordinator, the Heart Failure Data Coordinator is a RN who coordinates the Heart Failure Program data of Upstate Hospital patients who have a diagnosis of heart failure. Demonstrates leadership abilities, problem solving capailities, clear and effective communication skills, creative behaviors and the ability to function independently in a complex health care environment. Demonstrates the ability to sustain effective relationships with nursing and other disciplines.

Minimum Qualifications:

NYS licensed/registered (or eligible) RN, Bachelor's degree in Nursing and two years clinical acute care RN experience (preferably in Medicine/Cardiology) or Associate's degree in Nursing and fours years clinical acute care RN experience (preferably in Medicine/Cardiology) along with excellent analytical, written/oral communication and interpersonal skills required. CPR-BLS required by time of appointment.

Preferred Qualifications:

Experience with the "Get with the Guidelines" heart failure program data and data entry, heart failure standards and chart abstraction and data interpretation strongly preferred.

Work Days:

Monday through Friday with a few evening/weekend shifts for community education annually
