Job Summary:

The LPN Support Coordinator is an important member of the Inclusive Health Services Department providing supportive services to patients enrolled in Retention and Adherence Program (RAP) and People Aging with HIV (PAWH) Institute grant-based programs.

Main objective of this role is to assist patients in answering questions related to diagnosis, medications and provider recommendations. Assist patients in understanding their insurance benefits and completing necessary paperwork to ensure ongoing coverage. Assist patients in applying for benefits such as SNAP and SDI and discussion of budgeting to address financial shortages.

Key Responsibilities:

  • Identification of needs and barriers to accessing health care services.
  • Ongoing reassessment of needs and barriers.
  • Creation of an individualized support plan to address barriers and needs.
  • Continued monitoring of, review and adjustments to individualized support plan.
  • Administration of PAWH Screening assessment annually to grant focus population.
  • Referral to internal and external sources to address barriers to care and non-medical needs.
  • Collaboration with external agencies and with University Geriatrics.
  • Track patient appointments, ensure patients are aware and have transportation, routinely administer assessments at properly scheduled intervals, report stats to grants coordinator on schedule.
  • Participate in ongoing quality improvement activities.

Minimum Qualifications:

  • Bachelors Degree in Nursing, OR Associate Degree in Nursing with 2 years of experience, or equivalent combination of education and experience.
  • NYS LPN License
  • CPR-BLS

Preferred Qualifications:

  • 1-2 years working with patients with chronic medical conditions.
  • Experience working with a range of ages.
  • Strong personal organization and communication skills.
  • Previous case management experience preferred.
  • Knowledge of HIV, prevention and health care is preferred.
  • Experience working with BIPOC and LGBTQ+ Communities.

Work Days:

Monday-Friday 8am-4:30pm
