Job Summary:

The Orthopedic Nurse Navigator coordinates care ensuring timely access and seamless synchronization of services throughout the continuum of total joint care/health care system. Will provide medical information, education, and share knowledge regarding internal and external support services and resources and will facilitate coordination of those services. Promotes patient and family-centered care for ethical and informed decision-making in preparation for orthopedic procedures. Decreases fragmentation of care and promotes coordination of services. The role navigates patients and families through the joint replacement process/orthopedic continuum and will coordinate the efforts between surgeons, hospital administration, and staff while tracking the progress of the program and identifying areas requiring attention. The Orthopedic RN Navigator is responsible for the joint program management, report generation and review, process improvement, and patient satisfaction activities. The Orthopedic Nurse Navigator is responsible for tracking patients post-discharge including rehabilitation hospitals, assisted living, skilled nursing facilities, and those discharged home.

Minimum Qualifications:

NYS licensed/registered or eligible RN required. Bachelor's degree in Nursing or healthcare-related field three (3) years post licensure RN experience (preferably in specialty) or an AAS in Nursing and five (5) years post licensure RN experience (preferably in specialty) required. Excellent written/oral communication and interpersonal skills are required. CPR -BLS certification is required by time of appointment

Preferred Qualifications:

Three (3) years as a Certified Nurse /Patient Navigator in specialty area preferred.

Monday - Friday, Days
