Job Summary:

SUNY Upstate is seeking a highly motivated Advanced Practice Provider (APP) to provide care to Oncology Patient at SUNY Upstate Oswego Hematology/Oncology Clinic.

Working in collaboration with the attending physicians, APP will oversee Infusion for adult Hematology/Oncology as well as consult with physicians for abnormal findings as appropriate and assist in planning and ordering care and treatment for patient's requiring triage.

Key Responsibilities:

  • Provide comprehensive health assessments and develop plans of care for Hematology/ Oncology patients to include medical health  histories, physical examination.
  • Developing plans based on clinical diagnoses and problems, analyzing diagnostic testing and laboratory results, and ordering interventions based on clinical assessments.
  • Creation of Chemotherapy treatment plans, possible enrollment in clinical trials and care coordination needs.
  • Actively participate in quality improvements working within teams and committees.

As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.

Minimum Qualifications:

  • Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program) or will be eligible by time of appointment.
  • NYS Licensed/Registered NP or PA, or will be by time of appointment.
  • Current National Certification consistent to the clinical practice. 
  • Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
  • CPR-BLS Certification required at the time of appointment.
  • Must successfully pass pre-employment and credentialing clearances. 

Preferred Qualifications:

Hematology/Oncology Experience.

Work Days:

Monday through Friday, Days

4- 10-Hour Shifts
