Job Summary:
In this position, the Social Worker will be assigned to the Pediatric Surgery, Pediatric Gastroenterology, Pediatric Nephrology and Pediatric Rheumatology at Crouse Physician’s Office Building. Duties will include consultation through assessment, intervention, planning, and referrals to meet the psycho-social needs of patients and families. In addition, the Social Worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying helpful services to meet individual's health needs. The Social Worker works closely with the multidisciplinary team to enhance patient care. Candidate must possess communication skills, have knowledge of Social Determinates of Health and available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other Social Workers and back-up responsibilities.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry-level (SL2) positions at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of the limited permit.
Preferred Qualifications:
Previous knowledge and experience with the pediatric population.
Work Days:
Monday-Friday, days with occasional on-call/holiday coverage on rotation based on operational needs.
