Job Summary:

Function as a consultation and referral resource to address the psychosocial needs of the patient population served by Onondaga Nation Health Center. Through the use of a wide range of interventions and treatment modalities, contributes to the achievement of optimum health, satisfaction and effective care by addressing the behavioral, social and environmental issues which may impede health.

Advocate and facilitate access to community services. Assist patients with completion of forms needed to access community and social service programs, as well as medical intake forms. Provide crisis intervention services, as needed, and serve to bridge the gap for patients needing to access counseling services available in the area. Facilitate appropriate transition from the inpatient to the outpatient setting, with necessary services in place. Must have working knowledge of the spectrum of community resources that are available, as well as a repertoire of counseling and other psychosocial skills.

Minimum Qualifications:

MSW required. Ability to effectively communicate in both written and oral form with persons of varying educational and cultural backgrounds. Ability to problem solve effectively, in both crisis and planning situations.

Preferred Qualifications:

LMSW or eligible preferred. One year of post-Master's experience in a healthcare environment preferred.

Work Days:

Monday - Friday daytime hours

L2: $60,406 - $84,050

Please note the salary information shown is a general guideline only. Salaries are based upon candidate skills, experience, and qualifications, as well as internal equity, market and business considerations. The salary range depicted is based on full time, 1.0 FTE and would be prorated accordingly for less than 1.0 FTE.
