Job Summary:

Transitional Care Specialist is responsible for coordinating and implementing planned discharge support resources, ensuring that individuals receive the comprehensive support they require as part of their discharge plan. Connects patients with the necessary services and resources, such as medical care, social services, behavioral health support, housing assistance, and community programs. This may include coordinating and occasionally attending appointments, arranging transportation, and ensuring clients understand and follow their treatment plans. Will facilitate communication and collaboration among various healthcare professionals, family members, and other relevant parties to ensure a coordinated approach to care with the patient. Working with clients to develop skills and access resources that promote appropriate self-sufficiency and independence. Collecting data, analyzing outcomes, and evaluating the overall effectiveness of the care plan, escalating cases that require adjustments to plan. Must demonstrate strong organizational skills and ability to self-direct tasks to achieve goals, strong verbal and written skills to interact with clients, families, and a variety of professionals. A strong candidate will demonstrate commitment to improving the well-being of individuals and communities through addressing complex patient needs and promoting quality of life with diverse populations.

Minimum Qualifications:

Associate's degree in Human Services, healthcare, or related field, 3 years of professional experience.

Preferred Qualifications:

Bachelor's degree in Social Work or other human service/healthcare field preferred. Previous care/case management experience preferred.

Monday-Friday, Days
