Job Summary:

The Transitional Complex Patient Specialist works collaboratively with Case Management, Social Work, the treatment team, as well as other disciplines and community-based agencies and organizations to ensure smooth transition after hospital discharge for patients at high risk for readmissions and unnecessary Emergency Department visits. The Transitional Complex Patient Specialist will work with patients through the hospital encounter and after discharge through patient education, collaboration with community-based agencies and organizations, ensuring medical follow-up, and mitigating the negative effects of Social Drivers of Health.

Minimum Qualifications:

NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field, plus a minimum of three years acute care RN or acute care, transitional care, or chronic care RN/case management experience required. CPR-BLS certification is required by the time of appointment.

Preferred Qualifications:

Master's degree in Nursing or healthcare-related field and prior acute care case management experience preferred. General medical/surgical experience preferred.

Work Days:

Monday - Friday 8am - 4:30 pm
