The Diabetes Nurse Educator assesses, plans, implements, and evaluates the patient centered diabetes education program at the SUNY Upstate Joslin Affiliate. Plans and conducts continuing education program in diabetes for professionals and the general public. Collaborates with other educators and providers in the areas of program development, implementation, marketing and review of clinical programs/services. Participates in the development of education curricula and materials by writing original material and/or reviewing the work of others. Participates in case management, telephonic triage, and patient care planning and goal setting. Provide diabetes education services and general diabetes self-management education (DSME) to patient and families, care partners and community at Joslin and throughout the Upstate Network.
NYS licensed/registered (or eligible) RN, Bachelor's degree in Nursing, health or education-related field and 3 years of direct patient care clinical experience (preferably with diabetes care and management) required OR NYS licensed/registered (or eligible) RN, Associate's degree in Nursing and 5 years of direct patient care clinical experience required. Excellent written/oral communication skills and strong computer skills required. CPR-BLS certification is required by the time of appointment.
Master's degree preferred. Certified Diabetes Care and Education Specialist (CDCES) certification preferred at time of hire or within 4 years of hire. Ambulatory, and endocrine experience is preferred.
Monday-Friday, varying shifts between 7:00 AM-5:30 PM
