Job Summary:
The FFHC Case Manager is an important member of the patient services team in the Inclusive Health Services Department providing supportive services to patients enrolled in the Family Focused Health Care NYS Department of Health AIDS Institute grant-based program.
The FFHC program serves BIPOC women and birthing individuals living with HIV and are planning a pregnancy, are pregnant or serve as the primary caregiver for dependent children.
As a FFHC Case Manager duties will include but not limited to:
- Working with patients enrolled in the program to reduce health inequities by increasing access and acceptability of HIV primary care, sexual health, behavioral health, and supportive services.
- Facilitate appropriate and culturally relevant referrals to internal and community-based programs.
- Work closely with the medical team to provide excellent culturally appropriate patient-centered care case management services to address social determinants of health which affect health, functioning and quality of life outcomes.
- Track and follow up on referrals.
- This position requires excellent organizational skills, outstanding communication skills

Minimum Qualifications:
Bachelor's Degree and 1-2 years of HIV or other chronic illness related field experience, Or an Associate’s degree and 3 years of related experience, Or equivalent combination of education and related experience.
Preferred Qualifications:
Case Manager Experience.
Work Days:
Monday - Friday, 8:00 - 4:30
