Job Summary:
SUNY Upstate Pediatrics Department is in search of a Grant Funded Program Coordinator. The Program Coordinator will work closely with the medical team to ensure the provision of services to address social determinants of health which affect health, functioning and quality of life outcomes. The Program Coordinator works directly with the Medical Director to assure all criteria for contract funding is achieved and maintained.
Primary Responsibilities:
Develop, implement and evaluate operational activities for New York State Department of Health AIDS Institute and Health Research Institute, Inc. funded programs.
Implementation of workplan standards of care, fiscal oversight, policy and procedure development and data management system oversight.
Implementation of multidisciplinary, collaborative quality improvement activities aimed at providing excellent patient-centered care.
Minimum Qualifications:
Bachelors degree in Public Health, Nursing, Mental Health, Health Education, Health Science, Business Administration or other related field, with minimum of 3 years experience in health care and 2 years experience in program development and implementation including quality improvement.
Preferred Qualifications:
Master's degree in related field preferred.
Experience with grant funded projects preferred.
Work Days:
Monday-Friday, 8:00 am - 4:30 pm
