The Advanced Practice Provider will serve Full Time to provide care to patients within the Connect Care Clinic at SUNY Upstate’s Nappi Building at the Downtown Campus. The APP will provide management of patients recently discharged from the ED and hospital, as well as acute and primary care to patients in the outpatient setting.
Responsibilities include providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services, and maintaining outstanding relationships with families, referring physicians & all members of the health care team.
NYS licensed/registered NP or PA required. Master’s Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
CPR-BLS Certification required at the time of appointment.
1-2 years in Primary Care setting, Urgent Care or Emergency Department.
Monday - Friday, Days/Evenings with weekend rotation
