Job Summary:
Upstate Medical is seeking a highly skilled and motivated Advanced Practice Provider to join our team, providing comprehensive patient care for the Oswego Division of Hematology/Oncology!
Key Responsibilities:
- Collaborate with the Attending physician group and provide comprehensive health assessments and develop a plan of care for the patient population.
- Assess and evaluate patients, perform minor procedures, order and interpret laboratory tests & diagnostic studies, and arrange referrals, consultations, and therapeutic services.
- Document in the medical record data in a consistent, legible, accurate, concise and timely manner.
- Will provide patient/family education and utilize prescriptive authority working in collaboration with the Attending.
- Build and maintain strong relationships with patients, families, referring physicians, and all members of the health care team.
As an APP, you will be an integral member of the healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.
Minimum Qualifications:
- Master’s Degree as a Nurse Practitioner or Physician Assistant
- Current NYS Licensure or Registration as a NP or PA
- Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire)
- AHA CPR-BLS Certification required at the time of appointment.
- Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
Prior experience in Specialty areas preferred.

Monday - Friday, Days
