Job Summary:

The care management position is responsible for assisting patients and families in navigating their healthcare. Their primary focus is high-risk patient populations within primary care pediatrics. Each role will encompass patient care as well as being the lead coordinator for all services related to the assigned patients; working closely with multiple agencies, specialty services, and providers. This role will be responsible for the patient's individual needs as they relate to age, physical, psychosocial, cultural, and education.

Minimum Qualifications:

R.N. NYS license or eligible, Bachelor's degree in Nursing or healthcare related field, plus a minimum of three (3) years RN nursing experience required OR equivalent combinations of education and experience. CPR-BLS is required at the time of appointment.

Preferred Qualifications:

Master's degree in Nursing or a healthcare-related field. Pediatric and ambulatory experience preferred.

Work Days:

Monday - Friday with some flexibility between the hours of 08-1730
