Job Summary:

The Pediatric Ambulatory Clinical Case Manager's primary responsibility is to facilitate and coordinate patient care across the continuum to achieve optimal clinical outcomes and efficient utilization of healthcare services. This role focuses on providing comprehensive care coordination for pediatric patients in the ambulatory setting, ensuring that each individual receives the right care at the right time, in the right setting. It is essential that the case manager possess a working knowledge of the spectrum of hospital and post-discharge services that are available. 

Minimum Qualifications:

NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field, plus a minimum of three (3) years acute care RN or acute care, transitional care, or chronic care RN/case management experience OR Associates degree in Nursing and five (5) years acute care RN or acute care, transitional care, or chronic care RN/case management experience required. *If an Associate in Nursing, must be enrolled in an accredited BSN program with an anticipated graduation date within the next 2 years. Will be appointed temporarily until receipt of BSN. CPR-BLS certification is required by the time of appointment.

Preferred Qualifications:

Master's degree in Nursing or healthcare-related field and prior acute care case management experience preferred. Pediatric med/surg experience or ambulatory experience preferred; PRI certification a plus.

Work Days:

Mon-Fri with some holiday and weekend coverage
