This position will provide behavioral health services for employees and adult patients followed in the clinics at Nappi Wellness Institute. This position works collaboratively within the multidisciplinary team to provide comprehensive psycho-social management and support to patients and families. This position is responsible for all aspects of therapy and psychological assessment. Develops with the patient/family an individualized plan of care and modifies the plan as status changes. Coordinates care with PCP and various outside agencies and obtains referrals into additional programs to support the patient's recovery.
M.S.W., L.M.S.W., or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare setting and working with child/adolescent patient population) required. *New M.S.W. graduates who are LMSW, or eligible, will be considered for appointment to entry level (SL2) position at a commensurate salary. Should employment continue beyond 6 months, incumbent must become L.M.S.W. no later than by the completion of their first year of employment.
Previous clinical mental health care experience preferred. Prior integrated care experience is ideal.
